Emergency Evacuation
Integra Global health plans utilize ACM International to offer you and your dependents access to international emergency medical evacuation and other assistance services 24 hours a day, 365 days a year.
Hospital Admission Acceptance
In case of hospitalization overseas, ACM will facilitate admission by providing the hospital with verification of insurance cover and guarantee of payment through their 24-hour access number.
Direct Billing of Claims
Integra Global health plans can settle hospital bills directly both inside the United States and worldwide. And by utilizing our PPO network (First Health) inside the U.S. there is the added benefit of having all claims filing done for you.
Maternity Benefits
Benefits are as any other condition, after an initial 12-month waiting period (time-credit will be applied for prior coverage).
Wellness/Preventive Care Benefits
Generous Wellness Benefits for employee and covered dependents. These benefits are before deductible and cover extensive routine medical exams as well as immunizations for dependent children.
Vision Care
Annual maximum of $150 for routine eye exams, contacts,and corrective lenses.
Cover for Pre-Existing Conditions
A 6/12 existing provision will apply to entrants for medical coverage. This appies to any condition treated (including prescriptions) within 6 months prior to effective date. For coverage for pre-Existing conditions between 10 and 50 employees, condition is limited to US$4,000, (for less than 10 Employees, the $4,000 is not available) within 12 months of being continuously insured. Not withstanding the above, the program can waive pre-existing conditions where prior coverage is in effect.

Hence, should one develop a chronic condition you have the peace of mind to know that the health plan will cover your follow-up care, long-term treatment, prescription drugs, and office visits for the chronic condition. Worldwide Cover
Members have the freedom to choose within the area of cover where the want to receive such treatment. And they have the freedom to choose their medical provider.
Choices of two areas of cover:
- ">Plan I - Worldwide including the United States
- ">Plan II - Worldwide but excluding medical treatment received in the United States and its territories
Comprehensive Medical Cover
The plan pays for medically necessary treatment and services, both as an in-patient and out-patient. It covers for hospitalization, surgery, as well as office visits, prescription drugs and standard out-patient services such as consultations and testing.
Hospitalization
Inside the U.S. the plan pays a 90% benefit until one has met the $1,000 out of pocket limit and thereafter at 100%. Outside the U.S. the plan has a 100% benefit.
Covered expenses include:
- physician fees
- nursing care provided by the hospital
- use of operating and recovery rooms and equipment
- prescribed drugs and medicine for use in the hospital
- x-ray examinations, electrocardiograms, fluoroscopy
- blood, plasma and oxygen
- diagnostic and laboratory tests
- intensive care or coronary care
- private room and board
- hospital outpatient services, if necessitated by injury
- medical transportation
- charges made for treatment of biologically based mental illness.
In-patient & Out-patient Services
In-patient services includes any medically necessary treatment a member receives as a patient in a medical facility for which a member stays overnight. Out-patient services include medically necessary medical treatment members receive where they do not stay overnight. In-patient services are listed above under Hospitalization. Out-patient services are listed below and include:
- Physician visits, office visits and services
- Diagnostic x-ray and laboratory testing
- X-rays, radium and radioactive treatments
- Chemotherapy
- Prosthetic appliances, except hearing aids
- Examinations and treatments
- Minor surgery
- Minor injuries, cuts, burns and casts
- Pre-admission testing
- Anesthetics, injections and administration
- Presciption Drugs
High benefits
Integra Global group health plans typically pay 90% for most all treatment and medical services inside the U.S. and Canada. For services received outside the U.S. the plan has a 100% benefit.

Integra Global wants supports a healthy lifestyle for your staff. Hence, you will find generous Wellness Benefits included in the policy. And what's best these benefits are available before deductible.
Wellness Benefits include the following:
Child Immunizations & Routine Medical Exams : 100%
not subject to deductble for children from birth to age 18 for immunization against diptheria, hepatitis B, measles, mumps,
pertussis,polio, rubella, tetanus, varicella, haemophilus influenza B, and hepatitis A.
Child Preventive Care Services: 100%
not subject to deductible. Services include: health history, physical examinations, development assessments, anticipatory guidance,
appropriate immunizations and laboratory tests.
Child Preventive Care is subject to the following limitations:
- Child - Birth to age 12 months - $350 Calendar Year Maximum
- Child - 13 months through age 17 - $100 Calendar Year Maximum
- Child - Age 18 to 23 (only if full-time student) - $200 Calendar Year Maximum
Adult Routine Physical Exams: 90%
covered and not subject to the deductible for charges made for or in connection with the overall health and well being for Insured
and Spouse or Domestic Partner age 18 years and over. $500 Calendar Year Maximum.
Papanicolaou Screening Test: Treated like any other illness but not subject to deductible. Up to one test per calendar year for all
eligible females.
Prostate Cancer Screening: Treated like any other illness but not subject to deductible. One test per calendar year for males age 50 or over.
Mamograms: Treated like any other illness not subject to deductible per the following schedule:
- Ages 35-39: one baseline exam
- Ages 40-49: one exam every two years for asymptomatic women, but no sooner than two years after a woman's baseline.
- Age 50 & over: one exam annually
- Any age: Whenever prescribed by a physician.
Vision Care
Personal Health takes care of vision care needs as well. A $150 benefit every two years is available for routine eye exams, corrective lenses and contacts.
Pregnancy
Maternity benefits are some of the most generous in the industry. After an initial 12 month waiting period maternity benefits are treated as any other condition. Maternity benefits include covered pre-natal services.

Choice of Deductibles
Integra Global group health plans typically offer a choice of 5 deductibles each:
$100, $200, $500, $1,000 and $5,000.
You can decide what deductible option fits your company's needs best. The higher the deductible the lower the premium. All plans offer the
same great benefits. And Wellness Benefits are before deductible so your staff can stay on top of their good health.
The deductible is an annual calendar year deductible per insured. After the calendar year deductible has been met medical expenses
are paid/reimbursed at the rate indicated in the Schedule of Benefits.
Optional Dental & AD&D Cover
Optional benefits such as Dental, LTD, Life and Accident Insurance are available.
Flexible Premium Payment
Integra Global group health plans can be billed monthly, quarterly, semi-annually and annually. The policy is denominated in US Dollars.
